Most Pakistani fertility clinics rank for one or two procedure-name + city queries, and almost nothing else. They have a single landing page covering "IVF treatment" with the city name baked in three times, and that's the entire SEO surface. Three of the four search-intent types Pakistani fertility patients run are sitting un-targeted.
What gets missed: (1) privacy-aware local discovery queries: patients searching "private fertility center," "discreet IVF clinic," "fertility specialist near me" use a deliberately careful vocabulary that procedure-name landing pages don't surface for; (2) protocol-specific deep-research queries ("ICSI cost Lahore," "frozen embryo transfer Karachi," "PGT-A testing Pakistan"), captured only by clinics that publish a dedicated page per protocol, which almost no one does; (3) cycle-switch and second-opinion queries ("second opinion IVF Lahore," "fertility clinic after failed cycle Pakistan"), the highest-LTV intent type because these patients have already paid for cycle 1 elsewhere and are switching, and yet sit entirely un-targeted.
What we build is the opposite. A keyword map calibrated to all four intent types, dedicated landing pages per protocol (IVF, ICSI, IUI, FET, donor cycles, mini-IVF, fertility preservation), privacy-aware page titles and meta descriptions that win the careful patient's click without breaking PMDC rules, second-opinion / cycle-switch content covering the moment of greatest decision. Done consistently for six months, this routinely shifts a clinic from "ranking for one IVF query" to ranking on the first page across the four intent types rather than for a single query.
